INSTRUCTIONS FOR COMPLETING THE QUESTIONNAIRE

This questionnaire measures the type and amount of physical activity you typically engage in. The questions refer to activities performed in the last 7 days at school, for commuting, and in your free time.
For MODERATE physical activity, it is meant an activity that requires moderate physical effort and causes you to breathe at a moderately higher rate than normal (during such activity you would not be able to sing but you would still be able to talk).
For INTENSE physical activity, it is meant an activity that requires high physical effort and causes you to breathe at a much higher rate than normal (during such activity you sweat and cannot talk).
When answering the questions, consider only those activities that have engaged you for at least 10 consecutive minutes.
